West Bloomfield Health & Rehabilitation is a recipient of the Five-Star Quality Rating from the Centers for Medicare & Medicaid Services, or CMS. Created to help patients, their families and caregivers compare nursing homes, this system assigns an overall rating of one to five stars based on performance in three areas: state-conducted health inspections, nurse staffing, and medical quality measures.

In addition to individual ratings in each of the three performance categories, each facility is assigned an overall rating. The quality delivered by nursing homes with an overall Five-Star rating is very much above average.

To determine if a facility meets Medicaid and Medicare minimum quality requirements, trained inspectors following specific processes and assign Health Inspection Ratings based upon the last 3 years of onsite inspections, including both standard surveys and complaints.

Considering the individual level of care required by each resident, the Nurse Staffing Rating looks at the average daily hours of care provided to each. Facilities, where residents have more severe needs, would be expected to have more nursing staff.

The Quality Measure Rating encompasses eleven different physical and clinical measures based upon information collected by the nursing home. The Quality Measures provide information about how well a facility is caring for the residents’ physical and clinical needs.
